I’ve now spent a few hours trying to pair a brand new Leviton DZP03 dimmer without success. I have also not been able to pair my other dimmer (VRP03) either. For now, let’s start with what I have done to pair the DZP03.
- From the Hubitat Devices page, I selected “+” to add a new device and then “Add Other Devices”
- I first tried “ Find my brand”, selected Leviton, and then the Dimmer section, then selected the DZP6HD Smart Dimmer as the product type (based on reading other help posts)
- From the Hubitat app, I hit the “+ start Z wave inclusion” button and the inclusion mode countdown began.
- I then held the control button down on the Leviton for seven seconds until it flashed amber.
- I then pushed the Leviton control button once and it flashed Green continuously and then after a period of time it flashed red three times. Based on reading the instructions, this means the pairing was unsuccessful.
At this point, I reset the Leviton DZPHD to factory settings by holding down the Control button for 14 seconds. It’s successfully reset itself.
I then went back for steps 2-5 and tried “generic dimmer” with no success. I reset to factory default again and just tried “Add Device manually” and went through the process again with no success.
I have run into the exact same problem with the older Leviton VRP03 dimmer—cannot get it to pair. As it was previously installed with a Vera Plus, I was sure to exclude it first, and then to reset it to factory defaults.
I have successfully paired multiple other Leviton devices on my Hubitat without issue (Switches and dimmer switches) as well as some Jasco outdoor plugs. Trying to pair these Leviton plug-in dimmer switches has been highly frustrating and is driving me up the wall. It should not be this hard…..
At this point, I am Hoping wise members of the community can steer me in the right direction to get these things paired.

This is the easiest way to do it, the branded menus do the same thing except the provide you some instructions on how to put the device into pairing mode. Most people just use the manual inclusion and look in the device instructions.
I would do a shut down on he hub and unplug for 30 seconds.
After it boots back up, go to Settings > Z-wave details and grab a screenshot of the entire page there (may need multiple shots). Post screenshots please.

Your suggestion worked for the VZPPHD.
I shut down the C8 and then removed power for one minute and then powered it back up. I did a reset of the Leviton module and went thru the exclusion process on the C8 (nothing was excluded, but better safe than sorry…). I then followed the ”add device manually” inclusion process for zwave and it added to the C8 successfully (!!!!). I was able to name it, then assign it to a room, and test its function. 


Feeling lucky, I went ahead and tried the same process with the Leviton VRP03 dimming plug module (same zwave plus module, just older version). I reset, zwave excluded, and then zwave included it using the “add manually” process. I did not get a confirmation of inclusion, however. I then can see a new device listed in the hub’s Settings>Z-wave>Maintenance device list. I am providing a screenshot here showing the the device (which I believe is the Leviton VRP03.) as well as the just added. Leviton DZPHD.
Suggestions of what to do/try now? I have “discover” and “repair” as options for the device— this is my only device in the list with the Discover option. I see it does not have a device name like all the others do.

Ok first just FYI on the DZPD3 you might want to check out this driver:

For the VRP03 I think that is one of the very old devices that do not play nice with a C8, something with the zwave radio chip they do not like. Is that a Vizia RF device? It is most likely 300 series chip: https://products.z-wavealliance.org/products/832
Also, about your z-wave details. Any entries you have in there with no device name showing is what we would call or dead or ghost node. Sometimes on rare occasions if you click the discover button a couple times it may find the device and finish pairing but it very unlikely. How many of these ghost nodes do you have now with all the pairing attempts? Here is a guide with a lost more info: How To Remove Ghosts using hub tools or a UZB Stick
Here is a very old post where someone did get a VRP03 to pair, but based on the age it would have been a C5 hub is my guess, maybe even a C3. A lot of people have had issues with the old Leviton Vizia devices on the C8 (due to the 800 series radio we assume). Woes Pairing Leviton ZWave Dimmers

Thanks for the heads up on the DZPD3 driver. I have now added and applied it. 
For the VRP03, it is indeed one of the older Vizia RF+ devices. I may just relegate it back to my Vera Plus where it is comfortable and works fine. I need a lamp dimmer to replace it on the Hubitat so I’ll probably just end up getting another DZPD3-2BW.
Due to the unsuccessful inclusion attempts with the VRP03, I ended up with two ghost nodes. I followed the steps in the guide you linked and after unplugging the VVRP03 I was able to eventually get to the “remove” button using the native Hubitat tools and was successful at finally ridding the network of both. Is that the only old device you have an issue with? If you have multiple the other option would be to find a used c7 or c5 cheap and then mesh it with your c8. I think it would work on a c7 but not certain. Someone the other day was giving one away.
If it’s just the one device though probably not worth the hassle.
