If there are no ideas for troubleshooting that involve me continuing to use the ZEN16, then I do plan to move the two uses back to Konnected. I could then exclude it. But how would we know if the issue reoccurred? Note also that it was several months between the first two instances of the issue, but just days between the second and third.
Also, it seems that would leave out the possibility that something was sending the on to the parent from the hub, I.e., the driver, which remains a possibility.

We would recommend connecting the ZEN16 to something simple like a light that could stay on for a day so you could observe whether the device was tripped to on by itself.
Alternatively, you could move it back to a different Z-Wave hub and see if the issue remains, this would also help rule out Hubitat specifically as the source here.
Maybe I can chime in on this one. I have a Zen16 included into Homeseer connected to a linear actuator. I have not had this happen. Don't know if this helps or not.
