Is there a way to use a real tilt sensor as the open/close status in a virtual garage door? The only thing I can think of is just use rule machine to open/close the virtual door based on sensor status. Is there a better way?

No.
You could create a hybrid Virtual GDO which has a switch interlocked to the GDO elements. Your physical Switch can then alter the virtual switch via automation and you get Open and Close on the GDO.
