Well, I did a few more exclusions (hitting the zwave button 3 times quickly on the unit). I then unplugged it and plugged it back in. Then + device, enable zwave inclusion, hit the button on the unit again 3 times quickly, and it showed up. It also auto-selected this community driver that I installed yesterday. I don't think adding the community driver resolved the issue, but if someone else sees this, perhaps give this a shot.
