Dont worry about the dashboard yet, devices need to function from the device page before you worry about anything else.

I would shut the hub down and pull power for 30 seconds to reboot the radio.
If you are running platform 2.4.2 you may also need to go to zwave details and then click the "switch to legacy gateway" button. The new gateway Zwave JS IMO was a total mess on 2.4.2. You can always switch back to it once they work out more bugs.

The two that paired as "Device" probably did not fully pair, you can leave them enabled in SmartStart, and then exclude them. They should immediately try to re-join.

Z-Wave Issues

For partial or total loss of Z-Wave control (radio seems dead), before doing anything else please shut down the hub from settings and unplug for 30 seconds, then re-plug to boot back up. This will allow the radios to fully power down and restart. A regular reboot only restarts the platform and not the radio hardware.

What info to provide for troubleshooting assistance:

• How is Hub being powered? Supplied power block or other (explain)
  • PoE power (with splitter) is known to cause major Z-Wave issues on the C-8 hub.
  • See this post for detailed write up on PoE testing: PoE and the C8
• Provide screenshots showing entire Z-Wave details Page
  • Hub should be running for at least 12hrs without reboot, and state how long in your post (you can find uptime on Logs > Device Stats)
  • Click the Refresh Statistics button, wait 10 seconds, then Refresh the page
• Provide screenshot of Z-Wave Topology Map (red/blue squares)
