I just checked the Zwave log and have two orphan devices. I've tried to refresh each device, I've tried to remove and I tried a Zwave Repair. Both orphan devices are still there. The device list does not show any unnamed devices, nor is there any device with 28 or 30 in the DNI column on the device list.
How do I fix this?

The first one removed promptly upon shutting down hub, unplugging, leaving for 30 seconds, plug in again, refresh the device in the zwave log then delete.
The second one was far more stubborn. It did not remove despite over a dozen attempts at the above sequence. Also, I had no idea what device it might be a ghost of. I had the zwave log up on a tab and would just periodically go through the refresh/delete sequence and go on to other things. Concurrent with this process, one of my zwave battery powered sensors showed low battery, so I grabbed it and removed the battery and have it sitting here to remind me to install a new battery. The second time I did the refresh/delete on the ghost device with the battery out of zwave sensor, the ghost device finally deleted!!
