My Zen58 arrived so before deploying it, I bench tested it to learn how it worked. My interest was to replace the Zen16 which only operates 1 of the 3 contacts with remarkable unused current capacity. I learned it was possible to configure the Zen58 such that the status would report off when the relay had not opened: Opps. What I was trying to do was separate out the wired input for use as a status indicator for a wired contact sensor. Never could get that to work or create a child contact sensor. Then even though the specifications state 9 to 40VDC which I tried to power it off the safety sensor circuit of my door opener which measured 11.1 VDC beam unbroken and 12.1 VDC beam broken, the relay clicked but would not close the relay to operate the door control. Changing to the Zooz .5AMP 12VDC power supply solved the relay contact issue. Does it really work at 9VDC? Or did the little bit of current it pulled bring the voltage down to far. It did not interfere with the safety beam as the wall button worked as before. The Zen58 did replace my Zen16 as a Garage Door opener. In that the relay self opens after 1 second and the reporting does work as it should when configured correctly. To bad I was not able to eliminate the extra wall plug power supply and the associated wires. But the Zen16 hanging from a wire was eliminated so that is progress. Can it replace a Zen51. Yes. But at greatly reduced limitation for current.
The ZEN58 fills a good niche for low-voltage relay needs. Even though the ZEN51 has a "dry" relay contact that you could use for loads of any voltage, the reality is that, as a direct wire device, it needs to be placed in an electrical box, and the NEC says that you cannot have high and low voltage connections co-mingled in the same box. It would have been nice if the ZEN58 could also operate on 5VDC through a USB-C plug like the ZEN17, but I guess that would increase the size and make it too dissimilar to the ZEN57 from a manufacturing volume standpoint.
Has anyone gotten a Zen57 to work as a dry contact sensor? I changed param 5 to the contact sensor, and all the other settings as indicated in the FAQ, then excluded and reincluded, and the contact (on/off) child device isn't created.
Driver limitation, or am I missing a critical piece of the puzzle?

I was not able to get the Zen58 to do that. If the 57 is like it except for the power source then I doubt it. But when I bought it there was a disclaimer that the firmware was beta and may require a firmware update. So perhaps one day soon they will get that to work.

Driver probably does not support the advanced features. Does the system driver even have those settings or did you have to use something else to change them? If you had to switch to something else to change the settings then obviously the system driver does not support it.
The settings are there in the system driver, and the Zooz FAQ gives clear instructions, but...it seems like something isn't quite in place yet. I did see the answers above, but was hoping, someone had made a discovery! 
Odd, I would expect if the setting is there it should work.... so now I get the frustration.
I just loaded the driver on a virtual device, did you set the input to "Open/Close" or "On/Off Dry Contact"? The actual dry contact option is goofy, it sends the reports over on endpoint 2 (and in my testing also endpoint 0 which messes up the relay state). I would give that one the least chance of working correctly. I am fairly familiar with the device reporting because Zooz has me doing their SmartThings drivers, so that's where I have worked on it.
If you still have it setup you could turn on debug logging and then activate the inputs to see what gets logged. Inspecting the driver there is no extra capabilities other than switch stuff, so there is no hope of getting the states on the parent, it would have to make a child device. However checking the debug logs would show the device is in fact sending the reports to the hub, usually the system drivers will log all inbound reports.
Another good thing to know is, Zwave JS (C8 Pro only normally) or ZIP gateway?
We really need some input from @bcopeland on how its supposed to work.

This on my C8-Pro with Zwave JS, the device is joined using SmartStart and is in Long Range mode.
On/Off report (dry contact) -- and the other settings as below:
Interestingly even after changing these settings, and mashing the config button, nothing really changed in the behavior -- the contact is NC, so to test, I removed one lead from the connection, expecting the device to report "open" or "off". However, I got "button 1 pressed", and "button 1 held" events.
Upon re-reading the Zooz FAQ page on this device, I saw that it required a remove and rejoin from the network after those settings were changed -- so I excluded and rejoined it.
At that point, the state information changed to:
Yet, what should have happened (according to the FAQ), is that a switch device tied to the contact should have been created as a child, to show the state of the contact.
No child devices were created.
I think the device is configured correctly, and that is should be reading the state of the contact, but without the child device (as you noted) it has no way to report the changes.
I got home late this eve, and I've got to be up early -- but I'll try to generate some events with debug logging on and post them here tomorrow.

This is a really cool device, and if we can get the contact sensor working, there are a lot of things I would use this for -- although contacts aside, a tiny LR relay is pretty sweet too -- all on it's own.

If you are only using it for the inputs you could also leave it configured as defaults, and just use the relay state to determine the input state.
That's a good idea -- thanks! I might try that. You're right, I don't care what the relay is doing, as it doesn't control anything, so monitoring it's state might be an easy solution.... Essentially I was using a Zigbee Tuya GDO module to do just that, so there's no reason to conclude that this wouldn't work just as well.
One step at a time, I'll do the debug test first, and then decide from there.

Thats what I figured it would be. Seems like driver was just made quick and not tested for functionality. Its going to be at least a few weeks before I can even think about starting on this.
All good sir! At least we know that it works, and can be used as a dry contact monitor.

I just got my Zen57, but it says on the box "240V", not "120V".

Even though it's 240v instead of 120v, can I use this in North America?

Yes. Instructions say power supply is 100-240 V 50/60 Hz
That is my exact need as a "Dry Contact Sensor" just like a door open/close sensor I was going to hack.
Hopefully it works as advertised 
I got the Zen58 inputs to work for the "Dry Contact Sensor" needs
But I cannot get the relay to be FULLY independent by itself, I can disable the input from controlling but the relay itself cannot be controlled like a separate device.
What Am I missing?
I use a 58 for my garage door activator. Not using the inputs just z-wave. Works. What are you trying to do that is not working?
Dont use the dry contact sensor option, some really screwy stuff goes on when you use that option I found out. Ended up just not including it in the SmartThings driver I made for Zooz. Try doing it as just a "Contact Sensor" or "Open Close Sensor"
I have it working as far as my input sensing needs.
I was hoping to use the relay independently, However if I turn on or off the relay that also triggers the input which then triggers the rule I have running based on input state.
The relay would have to be controlled like a "Child Device" with a different name to allow what I want most likely?
No big deal as it works for the main purpose it was purchased for.....
Just would of been nice to utilize the relay fully independent of input
