Schlage BE469 lock left my wife outside the house, it died and HE never reported low battery, I was at home, so no issues.
@mike.maxwell is this something in my setup or driver issue? I'm using the BE469 driver.

Battery reporting on those locks is just plain shitty. When my BE469 was on Wink, the same thing happened several times. In the morning it would be at 90%, come home in the evening and the lock was dead.

I’ll second @aaiyar on this. Chances are good that the lock probably beep at you the last time you entered a code and nobody noticed. I’ve noticed that with mine. No battery warning but if the thing beeps after entering a code, you’d better change the batteries as soon as you take off your coat.
Cheaper to replace Schlage z-wave locks with zigbee equivalents. I just replaced a BE469 & FE599 with Yale YRD256 & YRL226 zigbee locks. It is incredible how much better they are (response time for locking/unlocking, reporting time for lock status).
Also make sure you DON'T use lithium batteries in the lock if you even want a chance at getting battery % numbers.
Lithium maintain voltage too long, and have too steep a death curve, to get accurate battery % in most cases.
I use Duracell Alkaline AA's in my Kwikset Zwave locks. With Wink, they would report battery percentage typically dropping by 10% increments. When they get to below 50%, they beep. I waited for them to get to 40% before changing the batteries … typically every 6 months. Since the transition to HE, my batteries are still above 70%, so I'll have to see how reporting goes.
