Is it possible to get the double press and hold options when using a lutron scene keypad (4-button pico with labels)?
Yes, this should be possible already with the Lutron Pico driver (unless you're running a very old Hubitat platform version--check Settings > Hub Details or Settings > Check for Updates).
Do note that you won't get this with the Fast Pico driver (it sends the events as soon as they happen--no time to wait for a double tap) or the legacy driver (which intentionally excludes this functionality for compatibility with setups that rely on multiple "push"es in a row).
How? My keypads don't work when I set them to "lutron pico" instead of "lutron keypad".

I don't have any of this specific device, but I have oher PJ2-4B models (one that just says that on the remote...not sure if that's the whole thing or if the long-lost box has the other clues), but I think they're all similar aside from the engraving. The keypad drivers are normally not intended for this kind of device, even though Lutron sometimes calls them that; anything that is a Pico should work with a Pico driver.
I second the advice above to check the number of buttons preferences (you want "4" for yours; it will give you errors for many of the buttons if not set correctly). If that doesn't help, see what Logs produces with this driver and one of the devs may be able to make sense of the error or adjust the driver if it fails with this one for some reason, though this is the most common problem.
Agreed AFAIK this is a Pico, not a keypad (Lutron makes other devices that are called keypads).

I figured out my issue. I was setting these scene type picos as keypads "k" when entering them in the Lutron Integration configuration list. They present buttons 8-11 which is what my rules are set up for. When I changed the type to Lutron Pico or Pico Legacy, the button numbers changed from 8-11 to 1-4 but I didn't update the rules to use the new button numbers.
