Very easy to reproduce.
Add linknd matter bulb to c8 using the mobible app. verify control of the bulb but then leave the bulb off. Remove power from the bulb and then return power (bulb will default turn on).
Now wait while hubitat reports the bulb as off, while the bulb is on. Rarely it will recover, but most of the time it will only recover if you repeatedly turn the bulb off/on from the device page.
