Actually, I modified your post to show the picture. You cut out 2 characters at the end, that caused the picture to not be displayed.

You do not need to fill out any of the categories. If you select (click) on "Lock", it should lock and if you scroll down you should see an "Unlock" button.

It is not locking or unlocking. Should I reset the door lock and try again

How did you add the lock to hubitat?
