3/4/6 are not valid devices. couple of them i removed through the zwave exclusion and the last one through device reset. cant seem to find a way to remove these entries though.

If you try to “reinitialize”, they should fail and give you the option to “remove”.
I have a few too that won’t remove. At one point a this one had a remove button that never worked. For weeks now it’s been stuck in initializing state. I opened a ticket and @bobbyD forwarded it to engineering.
This one says it’s alive, but the device has been sitting in a drawer for a week now.
I have this as well. Tried to remove and didn’t work even after re-initialize.
I have the exact same issue. These phantom devices have no way to be removed from the current system.
They seem to exist in the database since a z-wave stick reset does not fix this either.
Hoping Bobby responds here on this thread, once he has a solution to share.
@bobbyD? I have the same issue with at least 5 of these devices. Should I wait and watch this post or send in a ticket?
