What you're doing definitely sounds right, but I'm unable to find a manual for this product to confirm. It looks a lot like the Jasco model that was often sold under other brands like GE, so if you're able to find a manual for one of those, the firmware may be close enough that it could help. Unfortunately, most (all?) of those have also been discontinued now, so my searching didn't reveal any of those manuals. The closest I can find is for the new Honeywell Z-Wave Plus smart plug, which is obviously much newer than the Monoprice (which still has the old Z-Wave logo) but maybe they haven't changed much and it would be helpful. It still describes inclusion and exclusion as a single button press (promising that this didn't change, I guess) and a reset as holding the button down for >3 seconds as you plug the device into the wall.
Worth a try here. If it doesn't seem to do anything, I'd probably try that at various lengths with and without plugging it into the wall while held. If the manual doesn't say, I suppose it's anyone's guess. (I wouldn't give up on trying to get it to exclude, though--that way you'll have some confirmation it's ready to be included again; on some old GE in-wall switches, a factory reset still didn't erase the pairing information!)
