I doubt it is the HE radio database since all the devices are working fine and they respond properly to automations from Home Assistant.
That seems to be stepping outside of our expertise... Happy for others to chime in, but I feel @gopher.ny might be able to translate that better than us... Other dev's may also be able to assist

Do the backups run regularly and without error?
If yes, it's worth taking a manual backup and restoring it right back. Could be the database getting funky.
Have not tried back ups. I will try and let you know.
Something similar happened to me a few years ago. Downloading a db backup and then restoring it fixed whatever the issue was. This can also be done by going to “settings”, clicking “reboot”, selecting “show advanced options”, “rebuild database on reboot”, and then “yes”.
