Lock Code Manager: Cannot Resolve Duplicate Code Error

I am receiving the following log entry when attempting to add a code for a user.

"Garage Entry user code not added, duplicate[6:0xF]"

I have tried deleting the app, but I cannot resolve the error.
Can anyone offer any suggestions?

Have you tried deleting all codes via lock keypad? Then let LCM set them again.

Also do not try to use the master code (if you have one) as a valid unlock code in LCM...
