Physical changes of the switch/dimmer are not reporting a status update to the hub reliably. This often occurs when using old Z-Wave devices (i.e. non Z-Wave PLUS devices) OR WiFi devices that require polling to receive status updates OR Philips Hue devices that are controlled via the Hue app, instead of Hubitat (the built-in Hue integration does poll these devices every 60s by default which corrects the issue eventually, but not instantly.)
If your examples, can you clarify what "app device/light" is vs "device".
I assume the first "light is off" means the light is visibly off, but not sure what the other two mean.
Bulb is off, on my smartphone app tab lights/switches is show ON and under device bulb xxx current states is ON or sometimes off and doesnt correspond to the Lights/switches tab or phisical device
So in the app current states can be differents between tab lights/switch and connect to the hub/ device / bulb
Yes that makes sense now. For the purpose of troubleshooting, I would ignore the state shown on the Hubitat app for now. First just focus on getting the actual device state and the state on the device page in Hubitat to be in sync. Once that is working correctly, then I would move on to the app to see if it is still an issue.
First I would do a shut down of the hub, pull power for 30 seconds, then plug back in. This will allow the radios to power down and restart along with the hub.
To get anything out of sync to sync back up you should be able to toggle it on/off or refresh it. Then monitor it from there with a clean slate.
Have you added or removed any devices recently?
If you have a C7 hub, have you updated the Zwave firmware (Button on zwave details)?
I've looked at some of the replies below and one thing that I don't see explicitly mentioned might be worth calling out. SOME devices have their own way of switching the state they report. In our case these are switches that overlay currently rocker or toggle switches and mechanically turn the non-smart switch off and one. For this device we could "switch" on and off by pressing the switch button for 10 secs. Be sure you check the manual for the device to see if this is possible on your devices and they are just reporting backwards to what you expect.
