Flaky response on Z Wave devices

For the ones that are mains powered (e.g. switches) you probably need to remove power from the device that is the eventual successful inclusion. Usually that's the next device or two in the list.

Then do the refresh/remove.

ZWave Controllers like the hub have a list of dead (not responding) devices. Devices in the list (isFailed) are allowed to be removed. If the ZWave Radio SOC can 'ping' the device, then obviously it's not failed, and doesn't make it to the list, which means it can't actually be removed.

Getting a device into the isFailed list is the first step. Once there, the Remove button appears. When you click remove, it does yet another check for failed. IF the device remains in the isFailed list, it will be removed... otherwise the remove is ignored and you start over. It's for this reason that sometimes, powering off the device that was partially included, helps as @danabw suggests.
