Failing zwave device?

So I have a few of the Cooper zwave wall plugs. These are always showing INACTIVE every I go into the device page to look at them but I can turn them ON and OFF without issue most of the time. Sometimes they do act kinda weird. This morning I noticed that it was INACTIVE so I went to toggle it and I got these in the logs. What does it means? The device is bad?

They paired as Generic Zwave outlet.

I don't see any problem, just repeating commands, I have one Cooper outlet too, I have an external flood light connected and it works, I seen the inactive status on it too, the only difference is I'm using the generic switch driver instead outlet.
