For giggles (based on @bobbyd's comments) can you post your z-wave details page in it's entirety?
I reviewed your hardware for systematic failures. I didn't focus on the mix of devices, as that can only be done with your help. However, don't forget to update the Z-Wave firmware if you have problems on the Z-Wave side as well. The Z-Wave radio is different than Zigbee in symptoms and measures to resolve issues with devices connected to it.
Added a enbrighten zigbee switch about 3 feet from the sengled bulb in question. no change in behavior. When the bulb joins, it flashes as expected, then about 15 seconds later flashes red, green, blue again like it's just been reset.
Shutdown from the settings menu, unplug power to the hub (at the wall, not the hub) for about 40 minutes. This will throw all your zigbee devices into panic mode. Power the hub back up and see how it behaves. (BTW the sengled bulb is on constant power right?)
