Can't seem to create a the action after selecting the button. The button to add the actions comes up but does nothing when you click on it.

This seems to be only for the Iris Button Controller device.
What browser are you using? I've noticed similar behavior on seemingly random buttons/links in the UI with Firefox but only when I'm VPNed in, or maybe it's just a Windows thing (Firefox on Mac has never done this for me). I haven't figured out ant pattern, and Chrome still works when Firefox doesn't.
I was using Chrome on OSX.

What driver is this? It could be that number of buttons is not defined. Look at the device details.
I am using the built-in driver for the Iris Button Controller.. it's the single button version of the hardware.
What about the number of buttons setting? If it's 0, that would explain it. Anyway, why not just use RM for it?
There does seem to be some problem with this device, or rather with BC using it. Will investigate.
I did have a BC1.6 version that was using this device so I just edited that to do what I want for now.
I found the problem, There is a bug in the driver. Go to the device page and hit Save Preferences. You should see this for the device:
Once you do that, BC and RM-4.0 will both work for the device. You will have to remove any pre-existing app instances, they are broken due to no number of buttons when they initialize.
