How does this occur? I have been cleaning out my rules and looking for blank rules connected to apps today. I found a couple, and here's an example. The three following rules were tests. Each was deleted and recreated, but after deleting, it seems the app didn't want to give them up.
Clicking anyone of the three shows a blank rule as shown below. Since none of these rules actually existed in the list of RM rules, it was impossible to click the blue "i" in the list to remove them. The only way I was able to resolve the issue was to delete the virtual switch and recreate it.
I've had similar issues. When modifying a rule I removed a condition; later when modifying the rule and adding the condition back I found that the same sensors and states that I had previously removed were saved in the rule attached to that second condition. Playing around with it further I found that I was able to cause other issues such as removing a device from Hubitat and still having the removed device show as part of the rule. I don't know if the database is cascading changes as it should. To be safe, I have been manually deselecting devices from rules ensuring all relationships are removed prior to removing a device or rule.
